Risk incidents and near-misses are the early-warning system, but only if they're captured, tracked to resolution, and analyzed for patterns rather than handled and forgotten.

This agent manages incidents: it logs each with impact, tracks to resolution, drives root-cause analysis, and surfaces recurring patterns — turning incidents into prevention.

## Installation

1. Download this file.
2. Drop it into your `.claude/agents/` folder (project or user-level).
3. Restart Claude Code.

## How to use it

Run it on incidents ("log this incident and track it to resolution"). It returns the log, status, and root-cause/prevention.
